  • Your full name, date of birth, health fund details, Medicare number and relevant information from government agencies (such as Centrelink, Department of Veterans Affairs, Ministry of Health and/or the Accident Compensation Corporation);

  • Health information, including eye test results, other optometric records, diagnostic images of the eye, past and present health conditions, prescribed medication, optical prescriptions and any other health information provided by you or your healthcare provider;

  • Your payment details;

  • Information about your lifestyle, occupation/ employment and hobbies;

  • Details of your visit to our website and any transactions you carry out or forms you fill in on the website;

  • Any other information you have voluntarily provided to us.

We may also collect some information that is not personal information because it does not identify you or anyone else. For example, we may collect aggregated information about how users use our website

We are required to collect your name, address and date of birth as part of our legal and regulatory obligations, including under the Health Practitioner Regulation National Law as enacted in each state and territory of Australia.
